Medicaid

Medicaid is the state and federal partnership that provides health coverage for selected categories of people with low incomes. Its purpose is to improve the health of people who might otherwise go without medical care for themselves and their children. Medicaid is different in every state.

Freedom Health is a Managed Medical Assistance HMO (Health Maintenance Organization) providing services to dual eligible Medicaid Enrollees in the State of Florida with diabetes, chronic obstructive pulmonary disorder, congestive heart failure, and cardiovascular disease. Freedom 1st is the name of our Medicaid Plan. We currently provide services in the following counties: Alachua, Bradford, Citrus, Columbia, Dixie, Gilchrist, Hamilton, Hernando, Lafayette, Lake, Levy, Marion, Putnam, Sumter, Suwannee, Union, Pasco, Pinellas, Hardee, Highlands, Hillsborough, Manatee, Polk, Brevard, Orange, Osceola, Seminole, Charlotte, Collier, DeSoto, Glades, Hendry, Lee, Sarasota, Indian River, Martin, Okeechobee, Palm Beach, St. Lucie, Broward, Miami-Dade, and Monroe.
